Honesty note: no randomized trial has ever tested which ferritin level makes people feel best. The 50–150 band is where deficiency-treatment thresholds, symptom-trial entry criteria and safety data overlap — a reasonable target, not a proven one. Some clinicians dispute treating anyone in the "normal" range, and the WHO itself rates the evidence behind its own thresholds as low certainty. Menstruating women commonly sit at 20–40; men and postmenopausal women drift naturally higher.
Why am I exhausted with a ferritin of 30 my doctor called normal?
Because iron does more than build hemoglobin. Myoglobin in muscle and iron-dependent mitochondrial enzymes get depleted before anemia ever shows up on a blood count — this state is called non-anemic iron deficiency, and it is common in menstruating women, endurance athletes and frequent blood donors.
The best direct evidence is a 2012 randomized controlled trial in CMAJ (Vaucher et al.): 198 menstruating women with unexplained fatigue, ferritin below 50 ng/mL and normal hemoglobin took 80 mg of elemental iron daily or placebo for 12 weeks. Restless legs is even clearer: the international IRLSSG guideline recommends a trial of oral iron when ferritin is at or below 75 µg/L, and judges iron probably ineffective for RLS above that level. For hair loss, observational studies link ferritin below roughly 30–40 to increased shedding (telogen effluvium) in women, but there is no convincing RCT — the weakest of the three symptom links.
Practical translation: ferritin of 30–50 plus real symptoms is a legitimate case for a supervised iron trial and a retest, not a "your labs are fine" dismissal.
What is the optimal ferritin for women, athletes and vegans?
Menstruating women
Monthly blood loss makes iron the most common nutrient deficiency in this group, and ferritin in the 20s and 30s is statistically typical yet frequently symptomatic. The evidence-based action threshold is 50: below it, the fatigue RCT above showed benefit. Heavy periods, pregnancy plans or a vegetarian diet all push the case for testing yearly.
Endurance athletes
Hard training raises hepcidin (the hormone that blocks iron absorption) for hours after sessions, and runners add foot-strike hemolysis plus sweat and GI losses. The Sim 2019 review of iron in athletes notes ferritin below about 35 µg/L is a common screening flag, and that low ferritin blunts the hemoglobin-mass response to altitude training. Many practitioners dose iron in the morning away from the post-exercise hepcidin peak (roughly 3–6 hours after training).
Vegans and vegetarians
Plant (non-heme) iron absorbs at roughly 2–20%, versus up to about 25% for heme iron from meat, and is strongly modulated by vitamin C (up) and tea, coffee and calcium (down). Plant-based eaters run lower ferritin on average; the target range is the same, but supplementation is more often needed to reach it. Does high ferritin mean iron overload? (High ferritin but normal iron)
Usually not. Ferritin is an acute-phase reactant: it rises with infection, chronic inflammation, liver disease, heavy alcohol use, obesity and metabolic syndrome, and even hard recent training — none of which involve excess iron. This is why the WHO guideline recommends interpreting ferritin alongside CRP, and uses a higher deficiency cutoff (below 70 µg/L) in people with inflammation: inflammation can mask a true deficiency too.
The workup is simple. Repeat a fasting morning iron panel with ferritin, transferrin saturation (TSAT) and CRP. Per the ACG hemochromatosis guideline, elevated ferritin (above 200 in women, 300 in men) plus TSAT above 45% should prompt HFE genetic testing — and ferritin above 1,000 warrants specialist referral regardless. If TSAT is normal and CRP or metabolic markers are up, the high ferritin is usually mirroring inflammation or insulin resistance; in that case checking fasting insulin and ApoB tells you more than an iron panel will.
How do I raise ferritin — dose, timing and the every-other-day trick
- Dose: 60–120 mg elemental iron per dose (one 325 mg ferrous sulfate tablet = 65 mg elemental). More is not better in a single day — see below.
- Every other day, one morning dose: the Stoffel trials in Lancet Haematology showed an oral iron dose raises hepcidin for about 24 hours, blocking absorption of the next dose; alternate-day single doses gave significantly higher fractional absorption than daily or split dosing. A 2020 follow-up confirmed the same in iron-deficient anemic women.
- With vitamin C, maybe: classically advised (the RLS guideline pairs iron with 100 mg vitamin C), though recent trial evidence questions how much it adds. Cheap and low risk either way.
- Away from blockers: keep coffee, tea, calcium and antacids 2 hours away from the dose; empty stomach if your gut tolerates it.
- Expect a slow climb: in the Vaucher trial, 80 mg daily raised ferritin only ~11 ng/mL more than placebo over 12 weeks. Retest at about 3 months, and not during an illness (inflammation inflates the number).
- If oral iron fails or is not tolerated: IV iron through your doctor is the evidence-backed fallback.
Iron is the one common supplement you should never take blind: it accumulates, and men or postmenopausal women with mid-range ferritin have nothing to gain from "just in case" dosing.
